Trip Registration and Online Medical Form

A list of forms needed to register.

Thank you for your interest in participating in an ONDA trip.  This form will allow you to officially register for ONDA trips (please read ONDA's cancelation policy at the end of this form), and will provide us with pertinent medical and emergency information.

We ask for this information so that our trip staff will know in advance, of any special medical conditions you may have.  These questions are not used to screen trip participants; however, the leader may contact you to discuss whether the trip(s) you have registered for will be safe and enjoyable for you. This form is confidential, seen only by ONDA trip staff or medical personnel.

This form will need to be completed and re-submitted once a year in order to maintain up-to-date records.

ONDA Cancellation Policy
The ONDA field trips are a central part of our mission, and these trips are very popular with our membership.  Most of the trips fill very quickly, and we create a wait-list in an attempt to accommodate any cancellations.   However, last minute cancellations can result in trips that are not filled because there is not enough time to notify members on the wait-list of the opportunity to participate.   In addition, cancellations require additional staff time to reconfigure trip logistics, and in certain instances, last minute cancellations can result in a monetary loss to ONDA.

Accordingly, ONDA has adopted the following policy on trip cancellations:

1.     Trips listed as “free to members”.

While there is no monetary cost to members to participate on certain ONDA trips, the organization of these trips requires significant staff time.  Because most of these trips involve restoration projects, we want them to be filled in order to maximize our impact on the ground.   ONDA recognizes that summer is a busy time, and that life is filled with unplanned events.  However, unless there are exceptional circumstances, ONDA will strongly encourage members who are canceling within 15 days of a “free” trip to donate $25 to help offset costs of filling vacant spots, and to help us continue to provide this service to our members.

2.    Fee based trips.

ONDA offers several fee-based trips every year.   Should the trip be cancelled by ONDA or the 3rd party outfitter because of unsafe river conditions, inadequate bookings or for any other reason, all payments made to ONDA will be refunded in whole.

Because these trips are pre-paid in full by ONDA, if a member cancels a reservation, the following rules shall apply:

a.    ONDA will return a full refund for cancellations made more than 30 days before the start of a trip.

b.    ONDA will return a 90% refund for cancellations made between 30 days and 15 days before the start of a trip, PROVIDED that ONDA is able to fill that spot.  Should ONDA be unable to fill the spot, no refund will be made.

c.    There will be NO refund for cancellation made after 15 days before the start of the trip or for “no shows”.

At any time up to 3 days prior to the start of a trip, a member may propose a substitute participant for his or her place on a trip.   ONDA will not charge a substitution fee, but ONDA reserves the right to accept or reject the proposed substitution.    

The original registrant will be responsible for any outstanding payments still due on the trip, and the original registrant will be solely responsible for collecting any reimbursement of fees paid from the substituted participant.
